The Commonwealth Home Support Programme (CHSP) helps Australians live independently at home.

About the Programme

The Commonwealth Home Support Programme (CHSP) is a government-funded initiative intended for seniors needing entry-level allied health supports to enable them to live safely and independently at home.

Nimbus Health offers CHSP Physiotherapy and Occupational Therapy services to eligible clients living in Adelaide who are registered with My Aged Care.

Instead of paying privately (full cost) for our services, you are only required to pay a contribution towards each service you receive. These co-contributions are detailed below and paid via direct debit.

How to request

Requesting CHSP services with Nimbus Health is a simple four-step process.

Step 1. Request a Service

Complete and submit our referral form to initiate the process. Please ensure you provide your email address and phone number so we can contact you about your request.

Step 2: Eligibility Check

Ensure you are registered with My Aged Care while our  Care Coordination team review your request.

Step 3. Relevant Documentation

All co-contribution payments for our services are processed via direct debit. We'll send you a Direct Debit Authority Form as well as Service Agreement to fill out and sign. Please return completed forms promptly to us to ensure smooth processing of payments.

Step 4. Initial Appointment

Our Care Coordination team will be in touch with any questions and contact you to arrange an initial appointment. Once booked, we will send you a reminder prior to your initial appointment.
